Declaration of breaks or gaps in training or practice

Breaks or gaps in postgraduate training or medical practice of three or more continuous months must be accounted for when making application for registration with the College.

On the enclosed declaration form please provide details describing:

Any period of time since graduation from medical school where you have not trained or practised for three or more continuous months; and/or
Any break or gap of one month or more for health leave; any/or
Any break or gap related to immigration purposes.
